&lt;p&gt;I recommend that you first watch one of our webinars which can help you learn how to use the web form. The webinar can be accessed &lt;a title="Web Forms" href="http://www.youtube.com/watch?v=BBF9hFXEH-Y&amp;amp;feature=youtu.be&amp;amp;hd=1"&gt;Here&lt;/a&gt;. After you go through the webinar, I recommend that when you create the web form, you go through the following steps:&lt;/p&gt;
&lt;p&gt;1. Select &lt;strong&gt;Web Forms&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;2. Select the setting: &lt;strong&gt;Create Custom Web Form&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;3. In step 2 select the &lt;strong&gt;Customer&lt;/strong&gt; table and in step 3 select the &lt;strong&gt;Campaign&lt;/strong&gt; table&lt;/p&gt;
&lt;p&gt;4. After you select the website that will host the web form, make sure that one of the fields that you choose from the&lt;strong&gt; Campaign&lt;/strong&gt; table is called &lt;strong&gt;Campaign Type&lt;/strong&gt;. This will grab the &lt;strong&gt;Campaign Type&lt;/strong&gt; that you have created in Method.&amp;nbsp;&lt;/p&gt;
