For anyone following this, we deteremined that there were duplicate entries in the Entity table. If you are unable to use the fullname as the dropdown field, check for duplicates in the table field.

great advice. I've replicated some of the formulas in the basic Web-To-Lead form and can confirm that my call-back and quote web forms are linking the activity to the contact and working perfectly. I have put together a Word document with the formulas and some screenshots if anyone is interested.
