Hi Marco,
The Activity # field is really just the Activity table's RecordID field, it's automatically populated with the next value in sequence when you create an activity or work order.

That is correct. For the purposes of testing custom apps, I usually create a "launching screen" containing links to all my custom screens.

This depends on whether you want to redirect a stock screen to your custom screen (in the case of modifying a stock screen), or navigate to a custom screen from any other screen.

Try regenerating the table, or logging out and back in. If that doesn't work, you can call our support line at 1.888.925.6238 x.2 and we'll take a look with you. You could also just leave it there if it doesn't bother you, it shouldn't cause any problems.
