Have you done any screen level customization to your web form? The stock web form screen has validation for handling when a customer name is already in the database since the Name field is unique. This validation can be found on the hidden CreateCustomer button's click event.

Much of this will need to be done manually however, you can copy action sets from your other screens using the Show All Accessible Controls In Account option.
You'll want to be very careful when using this for different record types, as this could lead to logic / data validation issues.

Sorry for the late reply. The reason that this is happening is due to the nature of the address fields on the Activity table. These are linked via the Entity dropdown field. When a record is loaded from the table, the address values are simply pulled from the related record in the Entity table.
