What table is the Referral Source Entity field attached to? That's the table that would also need to be regenerated.
Did this help solve the problem for you? If not, then please follow my second suggestion and let me know what happens.

ok I created my report but now when I try and filter using the comments field in activity I get the following error.
Error^Action Sequence Order:1^ERROR:You have specified that a filter should be applied to your report, however no filter criteria was sent. For security purposes, this report cannot be shown.

I don't have a Referral Source entity table... Referral Source Entity is a dropdown in my Customer and Entity tables, and is not marked as unique in either of them.
Do I need to create one? It did not appear to be necessary for this to work in the Edit Lead screen.
I regenerated the Entity and Customer tables.

Regenerate the Entity, Customer, and Referral Source Entity tables (in that order). To regenerate a table go to Customize > Tables/Fields > click on the table name (e.g. Entity) > then click Finished Editing Fields.
