I was able to work through this with MisterBill. The email addresses that they were trying to register were found elsewhere in the database.

We did a test here and could not reproduce the bug. Can I get a screenshot of the Generate Report that picks up the report and stores it in an Action Report? It may be something there that is causing the error.
