Did you add another sales tax code in QuickBooks? If you did that then that code will be brought over to the Tax Code dropdown under the Sales Tax Information area in the Additional Info section within the screen under the Edit (Customer Name) tab link.

I have a very similar issue with copying the Cases screen and now when I go to the cases screen I also get an error with a Sequence Order.
