The way that you have created your Standard Web To Lead form was correct. The information that is filled out on the web form will be sent to the customers table. The contacts table is used to store information for additional contacts for the lead or customer so you don't need to send that information to the contacts table.

The Field Services Operations that I referred to is actually the app called Method Field Services for QuickBooks Desktop. if you are using QuickBooks Online, you will be using the app called Method Scheduling and Invoicing.

Got it. Then I'd like to request a feature enhancement that allows the user to maintain their own email account password for their own profile. This keeps the admin out of the loop, preventing them from dealing with their users' email password.

Hi Blake C,
As a security measure, the administrator has to create the users username, the initial password, and their individual email settings. After the user gets setup in Method, they can change their own password but not their e-mail settings.
