also i've looked in the area that sends emails under estimates, and i've gone to add new etc. and i see the tab for default pdf templates but nothing that shows how to actually edit that email template and include an attachment can you be more specific please? :)

What exactly is the error message that they are getting?
The payment types are populated from what you have setup under Customize > My Account > Payment Gateways

Something like this could be done using Portals.
You could create a customized Portal for surveys and send out that portal link to your customers and have them fill out that survey. That survey info would all be stored in a table which means you can then manipulate it as needed.
