Hi sbradley,
Judging from the error it looks as though there is a permission issue. Can you look at the Assigned To person for the web-form. Check in the Edit User wizard and see if they have a) access to the tables the web form is using and b) Access to the API in Step 8.

Your tables look fine. I believe you just need to alter your script a little bit. Don't use between and instead set two date ranges. Here's an example:
Customer_RecordID IN (100,101,102,103) AND (ShipDate>'Dec-01-2013 11:59:50 PM' AND ShipDate<'Dec-09-2013 00:00:00 am')

You will need to create a new customer that represents the vendor. In Method, you can do this by going to Customers > Customers and Contacts List and clicking New Customer.

My apologies for the late reply. Unfortunately you won't be able to link Work Orders to Vendor Bills since Vendor bills do not sync to QBO. This is why the Bill screen is hidden on a new Method for QBO account and also why you can't find the BillLineItem table in the tables list.

I took another look at this and realized this is actually the way it is designed. The time tracking entries are held back until that portion of the contract is invoiced.

You'll need to go to Customize > Tabs > Edit tab links for the Customize tab. When there, for "Screens" click Edit under "Roles that can view", click Edit. You'll see this screen:
Whatever you users role is, will need to be check here in order to view and edit screens.
