At this point it can be either outlook itself or the toolbars that are preventing you from seeing the Method CRM Add On. You can try un-installing and re-installing outlook to see if that works. The first one if you haven't done much customization to the "FieldService_AddEditWorkOrder" screen and don't mind losing the changes then try copying the screen from the account and you can then select Replace links to an existing screen in step 3 of the Copy Screen Wizard.
