I was able to replicate this issue on my end on my QBO account.
I have created a ticket for our development team to take a look and will send any updates I receive.

I discovered if I open the Loop first, then it is not a problem.

Still unable to replicate this error.
I have created a screen with over 12 fields, and multiple actions. The Loop value in a Insert to Table action does not lose its value.
Anything else I can add to my screen you think that I am missing? I'll give it another shot.
