The contact record must already exist before an activity can be assigned to it. You can first create an unassigned activity then manually assign the appropriate contact to them after they're created. Is this helpful to you? If not, what is your end goal?

Try this in Firefox, but first clear your browser cache/history. It shouldn't be happening but there is an issue with web forms on Google Chrome having their data reset as you mentioned, plus some other strange behaviour. One work around is to assign those actions to a hidden button on the screen then just call that in your grid and anywhere else you need to call it on the screen. Hope this helps!
