Just tested it on my account and I'm unable to replicate this. Left both the email and opt out checkboxes blank and selected only Active Customers. Do me a favor: Disconnect and shutdown your Method Integration Engine, then remove the data from those fields, and Save. Reload the screen after a few minutes, check the Sales Order you edited, and let me know whether the fields are populated again or not.

When you open your report, look at the Field List section, usually on the right side of the screen, and ensure you're using fields from the correct detail table. You want to use the fields from the InvoiceLine table and drag them to the Detail section of the report. Example below.

Just tested it using your QuickBooks_SalesOrder screen and it worked fine. Go to Customize > Screens and click the View button that comes up to the left when you hover over the screen name. If It's the latter then open your report and check the properties of each of the line item fields, i.e. item type, description, quantity, etc. You want to make sure that the padding or border is not set to a crazy number which ...

Ensure you are not editing any screens, Publish then Save & Close. Try regenerating your Sales Order table; just find the table and click Edit Fields..., when the edit fields screen pops up just click Finished Editing Fields. Are you making sure to Save your changes? There may also be some other validation rule you have setup, i.e. if Due Date is blank then stop processing more actions.
