This currently isn't possible. If you wanted to do something like this, you'd have to create multiple Web-To-Lead forms. You could have a dropdown on your website that toggles between them, handling the logic on that side.

We don't have an action for this, but you can use our Import / Export Tool to import data from a CSV file. Take a look at this Help Center article. You could give your users access to the API and the tables they need to use through it, and create a screen with a grid to show the information they're importing.

Sorry for the late reply. One thing you can try is to copy your custom screen and select Replace links to an existing screen, choosing your old Estimates screen. If that doesn't work, call in to our support line at 1.888.925.6238 x.2 and we'll be happy to take a look with you.

More specific to your situation, you'll want to check the filters for your Sales Opportunity dropdown (Step 5 of 6 for the dropdown). If you filter based on the Contact field rather than the ContactName field, it may resolve your issue.
