I think it would be possible but unfortunately I don't know how to go about it. The forms can be customized to look how you want but I'm not sure how you would grab the page info. Please bear in mind that we won't be able to support customized web forms.

You can actually do this now it’s just not immediately clear the option is there. Here’s the basics:
1. Select the dropdown from the designer
2. Scroll to the end of the properties panel (for the selected dropdown)
3. Open up the Actions/Events options
4. Add at least 1 action to ‘Custom Row Click’ ...
