Hi John,
Can you email me the Company Account name and activate the MethodSupport user? To do so, go to Customize>Users and click the Active check box next to the MethodSupport user. Thanks.

Hi Thomas,
Currently there is no way to upload files from ta web-to-lead form. You could set up a custom portal though that allows your customers to sign in and upload documents. Click here for an article about how to set up a portal and feel free to reply here if you have any further questions.

In the scenario in my previous post, I set my where clause as: where the RecordID is equal to the Value From Row, RecordID. Since I wanted to update every record selected in my loop, this was an appropriate choice.
