I'm not sure what mistake you made but if you want to see the original screens you can view them at the tab Customize > Screens. Can you describe what happened (or what's not happening) and I can try to help get the screens back for you.

The problem is iFrames, we use them but Safari mobile doesn't play nice with them. Best bet for Method on any iDevice is to use the mobile login.

Please go through the steps to create the form and test it inside Method - the last step, step 9 shows you how the form is going to look. Does everything look right here? Are you able to add an entry from here? If so then there might be an issue with the HTML on your site.

The original esimates page is called QuickBooks_Estimate and my copy is currently 13 days old. Everyone should have the same copy of these pages unless you're using an older version of Method.
