Hi Rodney,
To start, you should add a new field into the Customer table. To do this go to Customize>Tables/Fields. Scroll down to the Customer table and click Edit Fields.... The type of field you want to add is a DateTime field. Don't forget to click the Add Field button and then the Finished Editing Fields button when you're done. See my first screenshot below.
After you have added the field into the table, you can go ahead and place it on the screen. To do this, go to Customize>Screens. Find the screen called CRM_NewLead or CRMQBO_NewLead for Quickbooks Online users. In order to make edits to it you must first make a copy of the screen as you cannot edit stock Method screens. From here you can replace existing links to the stock screen, or put your new screen into a tab of your own. It's up to you.
Once you're in the screen designer, simply drag your newly created field from the Add Fields tab onto the screen. In your case, drag it to the Lead Information section (see my second screenshot below). If you need more help on how to customize, I suggest watching our webinars.
