The form was working correctly for the most part, however, we needed to make some changes to it as well as some of the tables that it relates to so that we could create a custom report (an employment application to print out) based on all this info. We realized we didn&amp;#39;t have some table relationships properly set-up to create the report, hence opening up this can of worms. I believe we have those relationships now set-up with the FullName field being the link between the Applicants, Employment, Employment Former, and Education tables (all the tables that are used for this web form). &lt;/p&gt;
&lt;p&gt;So in making all those table changes and then applying those changes in a new copied (Web-to-Applicant2) web form we are now getting a consistent error on the Web form (&lt;a href="http://moxie-girl.com/company/application.php"&gt;http://moxie-girl.com/company/application.php&lt;/a&gt;). After the required fields are completed it says there is an error and to contact the forms provider for a solution (&lt;a href="https://www.methodintegration.com/MethodWebForms/error.aspx?error=An%20error%20was%20encountered.%20Please%20contact%20form%20provider%20for%20a%20solution.%20Thank%20you."&gt;https://www.methodintegration.com/MethodWebForms/error.aspx?error=An%20error%20was%20encountered.%20Please%20contact%20form%20provider%20for%20a%20solution.%20Thank%20you&lt;/a&gt;&lt;a href="https://www.methodintegration.com/MethodWebForms/error.aspx?error=An%20error%20was%20encountered.%20Please%20contact%20form%20provider%20for%20a%20solution.%20Thank%20you."&gt;.&lt;/a&gt;)&lt;/p&gt;
&lt;p&gt;No information is sent to our method account from the web form when the error happens.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Would someone be able to take a look at this web form to see if there are any glaringly obvious code issues?&lt;/p&gt;

&lt;p&gt;I have an Employment table that I originally created with the field &amp;#39;Age 16&amp;#39;. I have since changed that to &amp;#39;US Legal&amp;#39;. it is a simple check box for yes or no (actually a radio button on the website that provides the value into Method). My web-to-applicant form does not show that field anymore in the form or in any of my tables. I am not real sure why the error was thrown.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;However, I must have a lot of errors b/c I am getting about 30 emails/day with people using the form and getting errors. Yikes.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Regarding the &amp;#39;Opportunity&amp;#39; field, yes, we are using that field for the Activity table (in two places - also for a follow-up activity). I believe this was setup by Method as I just copied that form. The formula still says like you stated - RecordID_ReadOnly_Step3 in both of those cases.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;I am not super familiar with the formulas format so that is challenging for me. It would be nice to have a formula list or something. Like for instance it would be nice to know how to say format a cell to show a date a certain way. &lt;p&gt;That time is probably about the time I noticed it messed up. Like I said it has happened before. I have noticed it happening when I close out of one form and then go into another and then back into the 1st one. not every time but sometimes it has screwed it up. I am not sure if that is the reason or not. In using Custom Forms it seems to revert back to the default tables when you start the Custom form process even though that is not what was saved. Hope this helps.&lt;/p&gt;

Hopefully after I go through all this work of creating the tables I need that when the Employee table is allowed to be changed by users that I can easily move these applicants into it (if they become employees) or easily take entries from applicant table and move them to the employee table checking them IsLeadStatus to yes to seperate them. I am sure I will need guidance with that when/if it happens&lt;/p&gt;
&lt;p&gt;I am seeing an isssue when creating the WTL forms, however. This has happened a few times now (I thought it was a glitch the 1st time but it keeps happening). I have multiple custom forms saved and certain forms (appears to be the bigger ones with more tables and fields) lose the information or won&amp;#39;t load all the information that was saved last. For example, I have a form called Web-to-Applicant. I created it with 4 tables (Applicants, Employment, EmploymentFormer, and Education), picked all the fields in the right order, formatted fields and formulas and saved it, which obvisouly took a decent amount of time. If I go back into that form almost all the fields are missing and it is looking to one table - customers, which I didn&amp;#39;t even direct it to initially. Can you help me figure out this glitch before I go through all the time of creating it and formatting this form again?&lt;/p&gt;
&lt;p&gt;Also another comment off this topic. While customizing and creating screens it will be really cool if we could preview the screens and how they look right from that area without having to publish them or save and close and then go view the screen. Just a thought.&amp;nbsp;&lt;/p&gt;
