Custom Web-to-Lead Form: How to populate the Name field from a combination of the First Name and Last Name fields

I'm creating a custom Web to lead form. How do populate the Name field from a combination of the First Name and Last Name fields. I assume I have to put a formula in for the "Name" field to pull in the other info from the other firleds when they are populated. What formula will do that? Also I want it to fill in the Last name followed by a space and then the First name.

Also can you set the default value for Opportunity Close Date to fill in the current date?

Set Default Filter for List View?

How do you set a default filter for a list view. I went in to customize the screen and edited the list grid. And navigated to the option to select the default filter and clicked on finish. Then I clicked on publish. Then save and close. Then I refreshed method, and still it defaults to the original default, sometimes it seems like it chooses a filter at random. I also confirmed that the list is set to my custom screen. Am I doing something wrong or is this not possible? Thanks

How do I use a copied screen in place of a default screen?

How do I use a copied screen in place of a default screen? I have already copied the screen I wanted to replace and edited the copy. How do I get Method to use the copied screen instead of the default screen?

Specifically, I am trying to add my quickbooks custom fields into the screens for adding a New Customer and Edititng or Viewing an existing Customer.

The note in Method within the Customer Screen says "Your custom fields are integrated with Method, but you will have to customize this screen and drag your Fields where you want them to appear."

So how would I do that, since the default screens connot be edited but only copied right? But when I copy them and try to add the customer fields, they still don't show up, even though I am selecting the "Replace links to an existing screen" option and selecting the default screen I want it to replace.

Do I have to also edit something else, like the table or something? I tried editing the Customer Table to use the copied screen, but that didn't work either.

How can I change the Opportunity Close Date field to NOT be a required field? If the default New Opportunity fields cannot be edited, how do I copy the screen and edit that Close Date field to not be required, and how to I replace the default screen with the copied screen?
