We got the problem figured out. The reason why your clients had problems submitting the web forms is because one of the servers that we have, had an invalid configuration and occasionally users which were submitting the web form, ended up using it.
We have it fixed now and made sure this doesn't happen again in the future.

Start > Programs > Method > Method Integration Engine for Use with QuickBooks.
Have you tried uninstalling and reinstalling the Method engine?

Yes, you can for sure leave .NET Framework 1.1, 2.0 and 3.0, since the problem seems to occur only in version 3.5 and version 4.0 of the .NET Framework.

Hey Mahlon,
HasBeenBilled is set when the time activity is added to an invoice.
This happens automatically and cannot be set if it's not attached to an invoice, my guess is that this part of the customization isn't necessary as it will be handled by QBO when it syncs.
Workflow:
- Attach time activity to invoice
- Sync to QBO
