Hello,
The Portal login page cannot be customized, you'll only be able to use it as is.
I'm not sure of your website structure but you can try adding another link beside 'Existing Customer Portal' for example, 'Contact Us' and redirect that to the webform.

Hello, recently we've had a lot of customers mistakenly use the "Contact Us" link at the bottom of the Contact Portal instead of submitting a lead through our web-to-lead form.
