After further investigation and research in Microsoft's knowledge base, we found that in some situations a service pack needs to be installed on computers with Outlook 2003, it seems to be a common issue. Try this and let me know if it works:
1. Download http://www.methodintegration.com/documentation/office2003_supportfiles.zip
2. Unzip the contents of office2003_supportfiles.zip to a temp folder.
3. Run extensibilityMSM.msi.
4. Run lockbackRegKey.msi.
5. Run office2003-kb907417sfxcab-ENU.exe
6. Uninstall the Method Outlook add in.
7. Close Outlook.
8. Reinstall the Method Outlook add in
9. Reopen Outlook.
