I recommend that you first watch one of our webinars which can help you learn how to use the web form. The webinar can be accessed Here. After you go through the webinar, I recommend that when you create the web form, you go through the following steps:
1. Select Web Forms
2. Select the setting: Create Custom Web Form
3. In step 2 select the Customer table and in step 3 select the Campaign table
4. After you select the website that will host the web form, make sure that one of the fields that you choose from the Campaign table is called Campaign Type. This will grab the Campaign Type that you have created in Method.
